The premium plan includes all basic plan services (DAC/USIS report, MVR, etc.) plus:

• List of every past employer over the past 5 years according to Government Records. Information includess:
• Name and address of the employer
• Wages earned from each employer
• Different employers by calendar year
• Job Verification for past 3-years. (Provide us certain information about your past three years’ employer(s) and we will obtain information concerning your job performance.)

Q. How long does the Government report take to get?
A. Usually the next business day. (Regarding the other information--driving, infraction/misdemeanor/felony, and SSN/Identity authentication—this information is typically available within hours. The “DAC Report” typically takes 15 days.)

Q. I was an employee for a person—but he paid me fees rather than wages and gave me a 1099 miscellaneous earnings at year end. Will this report show those earnings?
A. No. But, if you were a contractor, once you are a member you may opt to have DOTJobHistory obtain your 5-year history of payments and payees. (Incidentally, Owner/Operator jobs CAN show on the “DAC Report.”)

Q. Does the report show the employment periods for each company to the month?
A.No. The report shows employers by year. (Again, the “DAC Report” may contain this information.)

Q. Does the report tell what kind of job I did?
A. No. The report does not describe job duties (driving, mechanic, etc.) and does not provide job performance (satisfactory, terminated for cause, etc.) This information is however typically on the USIS DAC report (if your prior employer was with DAC) – which is included in this plan too. In addition, if the employment occurred during the past three years, you provide us the information and we will verify your performance—even if it is not on the DAC report.
